Not asking for a category up front

If the complaint type is buried inside the description, every complaint has to be read to be sorted. A dropdown at the top costs almost nothing and saves hours once complaints start piling up.

Letting the resolution go unstated

A customer can describe a problem perfectly and still not say what they want from you. Asking directly is easier to answer than guessing what "fix it" means later.

Treating every attachment the same

A photo of damage, a receipt and a screenshot all matter for different reasons. Labelling what the file is — "photo of damage", "receipt" — costs one extra field and speeds up triage considerably.
