You need to know where they are now so HR can find the right people to coordinate the handoff. Free text for current role fragments your view of the organization.

Letting the reason field be optional

A transfer without a reason looks incomplete to a manager. Mark it required. The answer might be "I want to grow into a leadership role" or "I'm relocating" — either way, that context matters.

No date field for preferred start

Timing is half the decision. Add a date field so managers can see whether they can absorb the move when the employee wants to go.

Can employees request a transfer to any department?

This form lets them request any department in your dropdown list. What happens next is up to your HR and hiring process — they contact the receiving manager, discuss timing and fit, and either approve or discuss an alternative.

Does submitting the form lock in a transfer date?

No. The form is a request and a starting conversation. The preferred start date is a suggestion; the actual timing is negotiated between the employee, their current manager and the receiving team.
