What goes wrong with this form

Asking for graduation date as free text
Use a date field so responses are sortable and you know whose graduation is when. Free text produces "May 2025", "spring", "2025" and comparisons fail.
Making the resume optional
You need the resume. Mark it required so applicants cannot submit without it — they will see the requirement before they type anything and submit.
Asking "why this internship?" without a text field
If motivation matters, add a textarea. That question as a yes/no or a single line loses the content you actually needed to read.
