Treating the application as a complete onboarding
An application form collects the membership request. The gym still handles billing, contract signing, waiver forms and facility orientation separately. Assuming a submitted application means membership is active is premature.
Membership type as free text
Use a select field with your actual available memberships — basic, premium, family, annual — rather than free text. A dropdown shows the applicant what is actually available and prevents mismatches like asking for a "family plan" when you only offer individual memberships.
Missing preferred start date
A start date field tells the gym when the applicant wants to begin. Without it the gym has to email to ask, adding delay to onboarding. A required date field on the application keeps momentum.

No. This form collects the membership application — which type, when they want to start, their contact details. Billing, contracts, waiver forms and facility orientation are handled separately by the gym. The application is the intake, not the complete onboarding.
