Yoga class registration form

A yoga class registration captures who is coming and what experience they have, so the teacher can pace the class and offer modifications. Health notes — injuries, tight areas, conditions to be aware of — help the teacher guide poses safely.

The form beside this text is live. Fill it in and submit it — it validates exactly as it would for a real respondent, and saves nothing.

Class or style as a select field

A dropdown of the yoga styles you teach — hatha, vinyasa, yin, power — lets students find the right class. Free text produces "the gentle one", "stretching yoga", "fast yoga", which is ambiguous and the teacher has to clarify.

Health notes as optional text, not a questionnaire

A single optional long text field for injuries or conditions lets the teacher know who needs modifications. A checkbox list cannot cover all possibilities; open text catches what matters for each student.

No card required.

How to build this form

5 steps in the editor. Nothing here needs a paid plan.

  1. Add short text, email and phone fields for the student's name and contact details.
  2. Add a select field for yoga style or class level — hatha, vinyasa, yin, power yoga — so students pick the right class for what they seek.
  3. Add a date field and time field for the session date and start time.
  4. Add a radio group for experience level: beginner, intermediate, advanced. This helps the teacher know the room's skill range.
  5. Add an optional long text field for health notes — injuries, tight areas, conditions to be aware of — so the teacher can offer safe modifications.

What goes wrong with this form

Specific to a yoga class registration form, not general advice about forms.

No health notes field at all

An optional text field for injuries or health notes lets the teacher know who needs a modification before class starts, rather than discovering it mid-pose. Skipping it means the teacher learns on the fly.

Class or style as free text

Use a select field for the yoga styles you actually teach — hatha, vinyasa, yin, power, etc. — rather than free text. A dropdown prevents mismatches where someone signs up for "the calm yoga" and shows up for power vinyasa.

Experience level as a text field

Use a radio group with clear options — beginner, intermediate, advanced — rather than free text. Structured choices are faster to read and let the teacher quickly assess the room's skill range.

Questions about this form

What is the difference between yoga class registration and permission slip?

A permission slip is for a child attending an activity and collecting guardian consent. Yoga class registration is for an adult student signing up for a class. The form for an adult is simpler and skips the guardian consent, focusing instead on experience level and any physical conditions.

Do I need to ask experience level?

Yes. The teacher uses it to pitch the class intensity and know what modifications to offer. A room of all beginners needs different pacing and language than a room of intermediate students; knowing this in advance shapes how the teacher leads.

Should I ask for medical history?

No. This form asks for conditions or injuries to be aware of for safe pose modifications, not a full medical history. The teacher is not a doctor and cannot diagnose. If a student has serious medical concerns, they should consult their doctor before starting yoga.
