What goes wrong with this form

Collecting personal tax IDs instead of business ones
Ask for a business tax ID or registration number, not a personal SSN. The form is for business onboarding, not personal identity verification.
No category dropdown for multiple supplier types
Without it, the CSV export has no way to filter vendors by what they supply, and you end up reading every row to find the right one.
Making the notes field required
Keep it optional; a vendor with nothing to add should not be forced to type something just to submit.
