Try it liveWhich business template should you start from?
These differ less in their fields than in what has to be true after someone submits. That is the useful way to choose.
If it lives on your own website
Start from the contact form. It is the one most often embedded, and the enquiry-type dropdown routes messages before a person reads them. Two lines of HTML put any of these on your page.
If the record has to survive being read later
Start from the incident report form. Date and time are split into two fields and severity into a fixed scale, so a report files while the details are fresh and still sorts months afterward.
If a job needs to be dispatched, not just logged
Start from the work order form. Location or asset and a routine/urgent/emergency priority are their own fields, so a request can be routed the moment it lands rather than read first to figure out where it goes.
If you are collecting named support, not a legal signature
Start from the petition form. Every reply is a named, timestamped answer with its own choice about being shown publicly — there is no signature field anywhere in this product, and this page says so rather than implying otherwise.
If a lead has to become a scoped project before you meet
Start from the client intake form. Service type and budget are both dropdowns, not free text, so a new client picks something you actually offer and a range you actually charge instead of typing a wish.
If it's a pledge, not a live charge
Start from the donation form. Amount is a number and fund designation is a dropdown, so pledges are countable and sorted by cause — and, like the payment request form, it collects intent to give rather than moving money itself.
If money should not be spent before someone signs off
Start from the purchase requisition form. Department, cost and urgency are all structured fields, so an approver reads one row instead of an email thread, and the CSV export sorts spend by budget centre on its own.
If the more specific templates don't quite fit
Start from the plain registration form. Its program dropdown is meant to be edited freely — it is the flexible option for a recurring workshop or club that is neither a dated event nor a single course.
If a client proposes the time and you confirm it
Start from the booking form. Date and time are structured fields so the CSV export sorts and dedupes cleanly — there is no calendar sync, so you still confirm each slot by hand.
If the only real requirement is receiving a file
Start from the file upload form. It is deliberately three fields — name, email, and a required file — because base64 storage is built for one or two files per person, not a bulk transfer system.
If you are onboarding a supplier, not a customer
Start from the vendor registration form. A business tax ID field and an optional insurance-certificate upload are what a client intake form does not ask for — this one is built for the other side of the relationship.
If a volunteer needs vetting before a shift, not just a slot
Start from the volunteer application form. Availability, skills and a reference-consent checkbox review a candidate before acceptance — the volunteer sign-up form is the lower-barrier version of the same idea.
If a party is booking a table, not registering for anything
Start from the restaurant reservation form. Party size is a number field for the same reason quantity is on the order form — a wrong type on this one seats the wrong number of guests.
If you are vetting members, not employees or volunteers
Start from the membership application form. A tier select and a bylaws-agreement checkbox are what a job application or volunteer application do not need — this one is for a club or association, not a job.
If it's full, not just busy
Start from the waitlist form. It is specifically for when something is sold out or at capacity — distinct from the sign-up-form's general early-access list, and the FAQ is explicit there is no automatic notification when a spot opens.
If it's a photographer, not a generic service
Start from the photography booking form. Session type and location are the fields the generic booking-form does not need — the same vertical-specialization pattern as the dental-patient-form.
If it's a lead for a distributor, not a customer
Start from the catalog request form. A physical/digital radio and a mailing address (used only when physical is chosen) are built for classic B2B lead generation, distinct from the demo-request-form's software angle.
If a pet has medication or feeding rules for the stay
Start from the pet boarding reservation form. Check-in and check-out are separate date fields, and the optional special-care textarea gives medication, feeding and behaviour notes their own place instead of a buried email.
If the price depends on the dog's size
Start from the dog grooming appointment form. Size band and service type are both dropdowns, so a groomer can see what to book straight from the row, and temperament notes go in an optional textarea.
If a garage needs the car details before the customer arrives
Start from the car service appointment form. Make, model and year sit in one required field, the service reason is a dropdown, and separate date and time fields state the slot the customer is asking for.
If a dealership wants to know about a trade-in up front
Start from the test drive request form. The vehicle, date and time are separate required fields, and a yes or no trade-in radio lets the salesperson prepare for that conversation before the visit.
If you are choosing between volunteers for a board seat
Start from the board member application form. Every candidate answers the same two required questions, on experience and on motivation, and can attach a resume, so comparing them means reading a column rather than different letters.
If requests reach you by email, chat and hallway conversation
Start from the project intake form. Deadline is a date field and priority is a dropdown, so requests sort by real due date and urgency, and an optional file upload takes the brief.
If a new client's details are scattered across emails
Start from the client onboarding form. Primary and billing contacts are separate fields and preferred communication is a dropdown, so what you need on day one arrives in a single row.
If funders compare requests by amount and timeline
Start from the grant application form. Amount requested is a number field and the project start and end are separate dates, so applications sort by size and timing. It does not verify nonprofit status.
If an adjuster needs the facts of an accident first
Start from the auto insurance claim form. Policy number, date, time and location are separate required fields and damage photos can be attached, so the first read gives the adjuster what a phone call would. It does not approve claims.
If a shelter screens applicants before a meet-and-greet
Start from the pet adoption application form. Residence type and own-or-rent are choice fields while other pets and the reason for adopting are written answers, so applications compare before you follow up. It does no home visit or check.
If a preparer wants the document list before the first call
Start from the tax preparation intake form. Filing status is a dropdown and the documents the client will bring are checkboxes, so you see what is missing early. Do not use it to collect tax documents or Social Security numbers.
If a privacy request needs a paper trail
Start from the data access request form. Relationship to your company and preferred delivery format are dropdowns and an acknowledgement checkbox is required, so each request is logged. Verifying identity and meeting legal deadlines stay with you.
If dispatch needs the address and problem type first
Start from the plumbing service request form. Issue type is a dropdown, the address is split into street and city, state and ZIP, and a photo can be attached. A plumber, not the form, provides the quote.
If a mover needs both addresses to estimate
Start from the moving quote request form. Origin and destination each have street and city, state and ZIP fields, alongside a date and a home-size dropdown. An estimator still writes the quote; the form does not price the move.
If the crew needs to know about pets and the type of clean
Start from the home cleaning service request form. Cleaning type, home size and time window are dropdowns and a pets checkbox is optional, so the booking arrives with what the crew must plan for. Payment is handled separately.
If donors give goods or services, not cash
Start from the in-kind donation form. A textarea describes the gift, an optional number field records an estimated value for the donor's own records, and a radio chooses drop-off or pickup. It does not issue a tax receipt.
If a prospect should say what they want help with before a call
Start from the financial advisor inquiry form. Areas of interest are checkboxes and assets are an optional dropdown range, so inquiries can be routed. It collects no account numbers and gives no advice.
If the technician needs the pest before the visit
Start from the pest control service request form. Pest type is a dropdown, with date and time fields and optional photos, so the field team can bring the right treatment.
If a yard job is priced by what is wanted
Start from the landscaping quote request form. Services are checkboxes, so one request can cover mowing and tree trimming, and optional lot notes and photos give the estimator a starting point.
If a job is big enough to need a bid
Start from the contractor bid request form. Project type is a dropdown, the budget range is optional and plans or photos can be attached, so a contractor can decide whether to bid. It creates no contract.
If it is a one-visit job, not a renovation
Start from the handyman service request form. Job type is a checkbox list, so one visit can cover several small tasks, and date, time and photos are included. For multi-week work, use the contractor bid form.
If a clinic needs the reason for the visit and the urgency
Start from the veterinary appointment request form. It opens with a notice to phone for a life-threatening emergency, then asks for the reason for the visit and an emergency yes or no.
If researchers report vulnerabilities to you directly
Start from the security vulnerability disclosure form. Type and severity are dropdowns, steps to reproduce are required and files can be attached. Reports are not encrypted beyond normal storage, so use PGP for sensitive ones.
If comments become part of a public record
Start from the public comment submission form. The proposal each comment concerns is a required field and speaking at the hearing is a radio, so comments group by item. It does not promise anonymity.
If volunteers choose between patrols and meetings
Start from the neighborhood watch signup form. Availability is a checkbox for patrol, meetings or both and the alert method is a dropdown, so you contact each volunteer the way they asked.
If a claim is about a house, not a car
Start from the homeowners insurance claim form. Damage type is a dropdown, the date of loss is a date field and photos can be attached, so an adjuster can triage water, fire, storm and theft from the row.
If a salon takes requests before it confirms slots
Start from the hair salon appointment form. Service is a dropdown, the preferred stylist and time are optional, and a new or returning radio is required. It sends a request; the salon confirms.
If a small hotel takes enquiries without a booking engine
Start from the hotel reservation inquiry form. Check-in and check-out are separate date fields, guest count is a number and room type is optional. It does not check availability or take payment.
If developers ask you for API keys
Start from the API access request form. A required use-case textarea and a company field help you filter requests, while expected volume and endpoints are optional. Keys are issued by a person, not the form.
If deletion has to be confirmed in writing
Start from the data deletion request form. A required checkbox states that deletion is permanent and the reason is optional, so each request records the requester's acknowledgement. Verifying identity and deleting the data is your process.
If a rental desk has to check age and licence before handing over keys
Start from the car rental application form. Licence number and state are separate required fields, a yes or no radio flags renters under 25, and a licence photo can be attached. It verifies nothing; your desk still checks the licence and approves the rental.
If drivers should record what they actually checked
Start from the vehicle inspection form. Areas checked are checkboxes, the safe-to-drive answer is a required radio, and defects and a photo are captured. It is a record, not a certified roadworthiness inspection.
If the enquiry could be about any of your services
Start from the quote request form. Service type is a dropdown, the needed-by date is a real date and the budget band is optional, so requests can be sorted in the CSV before you read them. A person still writes the quote.
If a change needs a risk level and a rollback plan on record
Start from the change request form. Change type and impact are dropdowns, and the reason and rollback plan are required, so each request arrives with the same evidence. Approval happens elsewhere; the form is the record.
If vendors respond to a bid and you compare them side by side
Start from the request for proposal form. It is the vendor's response: an RFP reference, approach, price and timeline are required, and the proposal document can be attached. Nothing is scored, and deadlines are enforced by you.
If shared equipment needs a named borrower and a return date
Start from the equipment checkout form. Equipment name, condition, checkout and expected return dates are required, and a responsibility checkbox is ticked. It does not track availability or remind anyone.
If a business needs to request materials from a vendor with quantity and unit price tracked separately
Start from the purchase order form. Add the PO number and vendor name fields for attribution, then quantity and unit price as separate number fields so your accounting can multiply them. Add a delivery date field to show the vendor when goods are needed; quantity and unit price are separate number fields, so a spreadsheet can multiply them from the CSV.
If customers need to request refunds by order number and reason, with optional damage photos
Start from the refund request form. Add order number and purchase date fields so you can look up the original sale, then a select field for the reason—damaged, wrong item, changed mind—which groups requests into trackable categories instead of free text variations. Add optional fields for a damage photo and refund amount; the form fills the reason field which groups your incoming requests by category without anyone reading descriptions.
If employees ask for approval to travel before booking flights, with departure and return dates separate
Start from the travel request form. Add destination and separate departure and return date fields so a travel coordinator can calculate trip length, then purpose and estimated cost fields. Add a dropdown for transportation mode and approver name; the form fills two date fields which let a coordinator schedule ground transportation and book flights without parsing a date range from one text box.
If a lender collects applicant details and income before a loan officer runs credit checks
Start from the loan application form. Add annual income and loan amount as number fields so a loan officer can compare against lending limits without parsing text. Add select fields for loan purpose and employment status which group applications by type and income source; the form fills number fields which a loan officer uses to assess debt-to-income ratio immediately without calling and asking.
If a local office needs permit type and project location to route applications to the right reviewer
Start from the permit application form. Add a select field for permit type—building, electrical, plumbing, sign—so you can sort applications by department in the CSV. Add property address as its own field so the office can check zoning in their GIS system; the form fills permit type which sorts applications without a clerk reading and categorizing every entry.
If a buyer sends a structured request to vendors asking them to bid on items and delivery
Start from the request for quote form. Add what is needed in a long text field, quantity and delivery date as separate number and date fields. Add an optional budget ceiling field; the form fills quantity and delivery date which vendors need to quote accurately without calling to ask how many units and how urgently they are needed.
If a new business owner applies to a local office with business name and type before verification
Start from the business license application form. Add business name and a select for business type—sole proprietor, LLC, corporation—so the office can apply the right regulations. Add owner name, address, and optional EIN field; the form fills business type which sorts applications without a clerk reading each description.
If an employee reports a tech problem with issue category and urgency so the IT team can prioritize the queue
Start from the it support request form. Add a select field for issue category—hardware, software, network, access—and a radio field for urgency level—low, medium, high. Add device affected and optional screenshot fields; the form fills category and urgency which let the IT team sort the queue by priority and type without reading each description.
If a front desk records who is visiting, who they are meeting, and why they are in the building
Start from the visitor registration form. Add visitor name and company fields, then a host employee name field so staff can verify the visitor against a calendar. Add visit date and purpose select field; the form fills host employee name and purpose which let front desk staff verify without reading a mixed description.
If an office admin collects booking requests with room name and exact time window so they can check availability
Start from the meeting room booking form. Add room preference as a select field and separate start and end time fields so an admin can see the exact window without parsing. Add number of attendees and optional AV equipment checkbox; the form fills start and end times which tell an admin the exact duration without a callback asking when the meeting ends.
If a professional services firm needs to qualify which inquiries are worth a callback
Start from the consultation request form. Collect name, email and phone so you can reach the prospect, then capture area of interest as a dropdown and preferred contact time so every callback is already scheduled. The limit: this form takes the inquiry; a person still calls back to confirm the meeting — no calendar is built in.
If a support team needs every ticket to arrive with category, priority and description so triage does not require reading
Start from the help desk ticket form. Use category and priority as dropdowns so tickets sort themselves before anyone reads them, and a required description field so no ticket arrives empty. The limit: the form creates the record; a person still routes it to the right team — no automatic assignment happens.
If legal needs to draft an NDA but cannot start without knowing whether it should be one-way or mutual
Start from the nda request form. Capture purpose of disclosure as a required text field and nda_type as a required radio choice so legal has the fundamentals before drafting starts. The limit: this form requests a draft; it is not the agreement itself — signature and final terms happen separately.
If a franchisor needs to screen inquiries by location, available capital and business background before sending materials
Start from the franchise inquiry form. Use location as a dropdown to spot regional interest clusters, capital available as a select range so the franchisor sees who meets the investment minimum, and experience as a required text field. The limit: qualification happens here; financial verification and territory approval still happen in conversation.
If a contractor needs a site address, time window, visit reason and access instructions before leaving the office
Start from the site visit request form. Collect address, preferred date and visit_time so the contractor arrives at the right place ready to work, and reason for visit as a dropdown so they bring the right tools. The limit: the form books the intake; a person still confirms the appointment — there is no calendar integration.
If support needs to find a customer's warranty and ownership record by product model and serial number
Start from the product registration form. Use model_number and serial_number as separate text fields so the CSV export is searchable by product type, and purchase_date as a required date field so warranty eligibility is never a dispute. The limit: registration records ownership; it does not activate warranty — that happens when support checks the registration.
If a PR team needs to see every reporter's deadline and story angle before deciding how and whether to respond
Start from the media inquiry form. Capture deadline as a date field and topic as a required text field so your PR team sees the publication timeline and angle without a callback. The limit: this form routes the inquiry; a PR or comms person still decides whether to comment — no automatic response is generated.
If an affiliate manager needs audience size, promotion channels and website proof before approving a new partner
Start from the affiliate application form. Use audience_size as a select range so you can instantly filter by reach threshold, and promotion_methods as a checkbox group so you see which channels each affiliate plans to use. The limit: approval and payout terms are decided by a person reviewing the application — underwriting is separate from intake.
If a sales team needs the account number, current expiry date and requested changes to prepare a renewal proposal
Start from the contract renewal request form. Collect account_number so the right contract is found instantly and current_expiry as a date field so the sales team knows how much runway exists. The limit: the form starts the conversation; renewal terms and signature happen outside the form.
If business development needs to route partnership proposals by type and know what is being proposed before taking a call
Start from the business partnership inquiry form.
