Collecting a second contact name without clarifying why
If you ask for a billing contact, explain that it helps you send invoices to the right person. Otherwise it feels redundant.
Using a textarea for goals when a phone call would be faster
Long-form goals are better discussed live. Use the textarea for follow-up clarification or to document what was already discussed verbally.
Forgetting to ask about preferred communication upfront
Assuming all clients prefer email leads to missed messages if they prefer phone or Slack. Ask explicitly.

Should I send this form before or after the contract is signed?
Send it after the contract is signed. Onboarding assumes the deal is closed and the client is ready to start work.
What if the billing contact is the same as the primary contact?
Leave the billing contact field blank and note in your process that they are the same.
Can I use this for both B2B and B2C clients?
Yes, but simplify the company/billing section for B2C. Remove or make optional the fields that don't apply to individual clients.
