This form has no built-in anonymization. If you use it over HTTP or on a network you control, your IP address and the timestamp may still identify you. For truly anonymous surveys, use a dedicated survey platform.

Can I make the survey anonymous by default?

Remove the name field entirely if you don't need it. Be aware you then lose the ability to follow up with individual respondents.
