Equipment checkout form
An equipment checkout form documents who took what and when they promised to bring it back. This is not inventory tracking — nobody needs a reminder to return something, and a form cannot schedule or check availability. What it does is create a record so you can see at a glance what is out and whose desk it left from.

Two date fields separate checkout from return
Checkout date and expected return date as two separate fields means the loan window is clear and the form can be sorted by return date to see what is overdue. A single date or free text cannot show who is currently late without reading every row.
Condition at checkout protects against disputes
Recording the item condition when it leaves — excellent, good, worn, damaged — means a scratch that appears by return cannot be blamed on the borrower. The form is the baseline, and CSV exports preserve the history.

What goes wrong with this form

No return-date field
If the expected return is absent or free text, you cannot sort the checkout list by what is overdue. Two date fields — checkout and return — is what makes a CSV list sortable by who is currently late without reading every row.
Recording condition as optional
Condition at checkout is the baseline for returns. If it is optional and nobody records it, a scratch at return becomes a dispute. Make it required and always record the state the equipment left in.
Expecting the form to prevent borrowing the same item twice
The form does not check real-time availability. Someone can check out the same projector twice in one week if they fill the form twice. The checkout list in your CSV is what tells you something is double-booked, and you have to follow up by phone.
Questions about this form
Can the form track whether equipment was returned?
No — this form records checkout. A return would be a separate form or a note in a spreadsheet. The checkout form creates a record of who has what and when they said they'd bring it back; someone has to note the return separately to close the loan.
Can I prevent someone from taking equipment that is already checked out?
No. This form creates a checkout record, not real-time inventory tracking. If preventing conflicts matters, keep a shared spreadsheet or use a dedicated asset-tracking system. This form gives you the data after the fact.
Should I ask for a reason why they need the equipment?
Asking for the purpose (research, testing, setup for client meeting) is optional and useful for context, but does not affect the checkout process. Keep it optional so a quick loan is still quick.
