Expecting the form to collect or forward the letter
This form requests a recommendation; it does not carry the letter. A teacher writes the letter outside this form according to the program's own submission process. Asking a teacher to paste a letter into a form field is optional and not part of this template — the real submission goes to the program directly.
No deadline field, so teachers do not know when the letter is due
A date field for the deadline is critical. A teacher with three recommendation requests cannot prioritize without knowing which is due first. The deadline is what makes one request "urgent" and another "you have time for this one".
Asking multiple questions in one textarea instead of organizing them

How does the teacher actually submit the recommendation letter?
They do not submit it through this form. After receiving the request, the teacher writes the letter according to the program's own submission instructions — often directly to an admissions portal or to a specific email address. This form is how a student asks; the teacher handles the writing and submission in their own workflow.
Can I ask the same teacher for letters for multiple programs?
Yes. Submit the form once for each program, so each deadline is tracked separately. A teacher receiving three requests can see that one program needs the letter by March 1st, another by April 15th, and keep them organized without mixing up deadlines.
What if a teacher wants to send a letter directly to me instead of the program?
That is between the student and the teacher. Some programs allow submission from either the applicant or the referee; some require it from the referee. The teacher should follow the program's instructions, and the student coordinates with them on the method.
