A homeschool co-op enrollment records a student's profile for its own records. This form is an intake for the co-op's planning and communication, not a state filing. Parents remain responsible for any homeschool registration their state requires; this form is what a co-op collects to organize classes and send curriculum information.

Grade level in a dropdown, not a number field
A dropdown with clear labels — Kindergarten, Grade 1, Grade 2 — is what a co-op needs to know. A number field produces "1st", "1", "Grade 1", "Year 1", all of which mean the same thing but require interpretation.
Multiple subject interests via checkbox group, not radio buttons
A student is often interested in several subjects; checkboxes let one enrollment express multiple interests. Radio buttons force a single choice, which loses the full picture of what a student wants to explore.

What goes wrong with this form

Collecting state homeschool registration status
Do not ask families whether they are registered with the state or whether their status is current. The co-op is not responsible for state compliance; that is the parent's responsibility. Your intake is for the co-op's planning, not for auditing family compliance.
Making curriculum a required field instead of optional
Families use different curricula or none; some switch mid-year. An optional field lets families note what they use if they want to, while an optional text note is better than a required dropdown or required text field that forces an answer.
No subject interest field, so classes get planned by guesswork
A checkbox group for subjects lets you see at a glance how many students are interested in science, math or history. Without it, you are left asking families after they enroll, which delays planning and leaves some interests unmet.

Is this form a substitute for state homeschool registration?
No. This form is for a co-op's own intake records. Parent-teach organizations and learning co-ops collect this to organize classes and send updates. Your state may require separate homeschool registration; check your state education department's website. This form does neither the filing nor the verification.
Can I require a specific curriculum for enrollment?
You can note a curriculum preference in the form or in your enrollment policy, but the form cannot enforce it. Add a curriculum field as optional if you want to know what families use; if your co-op requires a specific curriculum for a class, that is part of the class description and enrollment criteria, not a form field.
