Training registration form
A training registration form is an enrollment tool, not a tracking tool. It captures who signed up, which session they want, and any accommodations they need. You still manage the session itself — roster, attendance, materials and certificates (if any) are handled separately.
The form beside this text is live. Fill it in and submit it — it validates exactly as it would for a real respondent, and saves nothing.
Dropdowns show available sessions without explaining each one
A dropdown of training titles lets employees pick in one tap. Free text produces typos and repetition; a dropdown is clear and sortable.
Accommodations collected upfront
Asking about dietary or accessibility needs on the form means you have the information before the training starts. Waiting and asking the day of leaves less time to prepare.
No card required.

5 steps in the editor. Nothing here needs a paid plan.
- Add text fields for the employee name and email.
- Add a text field for department so you can route questions to the right people.
- Add a dropdown listing your training sessions so employees can select the one they want.
- Add a date/time field for their session preference if you offer multiple time slots.
- Add an optional textarea for dietary or accessibility needs so you can prepare the room and materials.

What goes wrong with this form
Specific to a training registration form, not general advice about forms.
Letting session choice be free text
Free text produces typos and variants of the session name. A dropdown ensures everyone picks from the real list of available trainings.
Making dietary or accessibility needs optional when they matter
Mark this required so you capture every need. Someone might not mention they need a quiet space or a dietary accommodation if the field is optional.
Treating the form as proof of completion
Registration and completion are different. Someone registers and then does not show up. Actual completion — who attended, what they learned — is tracked by your training team, not this form.
Questions about this form
What happens after I register?
You receive a confirmation (if the form sends one). HR or the training team gets your registration and manages the rest — roster, materials, attendance and any certificates. This form collects your interest; tracking completion is handled separately.
Can I change my session choice after registering?
Contact the training coordinator or HR. This form is the initial signup; changes after that are handled by email or through them.
Does this count as training completion?
No. Signing up is not the same as completing the training. HR or the training team track actual attendance and completion separately, using their own system or roster.
