What goes wrong with this form

Asking for availability as free text instead of dates
Free text like "anytime in December" is not searchable. Date fields let you sort applicants by when they can actually start and how long they can stay.
Only allowing one position choice
Use checkboxes so applicants can say they're open to cashier OR warehouse. A strict single-choice radio misses candidates who are flexible.
Not collecting contact info clearly
You need their phone number, not just email. Seasonal hiring moves fast and a phone call is how you reach someone same-day. Collect both and prefer phone for outreach.
