Duration in a dropdown, not a freeform field

Number of positions as a number field, not text
A number field prevents "a couple", "2-3 people" and "as many as you have" — staffing decisions are made on exact headcount, which a number field enforces rather than requiring the coordinator to guess intent.

What goes wrong with this form

Asking for preferred shift or hours instead of start date
Start date and duration are what a staffing desk needs to know first — preferred shift is secondary and comes after the role is identified as fillable. Ask start date, duration and required skills; let the desk handle whether second or third shift is possible.
Making skill requirements a checkbox group instead of free text
Skills and certifications rarely fit a predefined list — a role might need "Salesforce admin", "ASP.NET 6+", or "OSHA 30-hour cert". A textarea lets managers state exactly what they need without you maintaining a master skills list that is always out of date.
No duration field, so it defaults to unspecified
A request with no stated duration looks the same whether you need someone for four hours or four weeks. A dropdown with six clear options is what lets a coordinator prioritize same-day requests ahead of month-long ones without opening each request to read the description.
Questions about this form
Should I collect the specific start time as well as the date?
If same-day staffing is common, add a time field after the start date. A recruiting desk that handles day-of requests needs to know whether you need someone at 6am or 9am. This template leaves it as a date for day-in-advance requests; add the time field if your needs differ.
Can the same manager request multiple positions in one form?
Not in this template — each form is one request. If a department needs three roles filled at once, three separate requests arrive timestamped within minutes and can be grouped by the receiving desk. A multi-line request loses clarity on which skills go with which position.
Where does a staffing request go when submitted?
Into your responses list first, always — that is the record. From there each request can also be sent to a webhook (your staffing system), Slack, Discord, email or Teams, so the recruiting desk sees it without opening the dashboard. Add a destination in Settings when you build the form.
