UX research session signup form

A UX research session — whether an interview, a usability test, a tree test or a card sort — needs participants with the right device and the right time. This form collects device, browser, session type interest and availability windows so your team can schedule without back-and-forth. The same shape works for any session type; only the session dropdown changes.

How to build this form

6 steps in the editor. Nothing here needs a paid plan.

  1. Add name and email fields for identification and invitations.
  2. Add a dropdown for session type — "Which session type?" with Interview, Usability test, Card sort, Tree test, or Moderated feedback. If there is only one session type, you can skip this.
  3. Add a dropdown asking "Which device or browser do you use most?" — iPhone, Android, Chrome, Firefox, Safari, Edge — so your team knows what to test on.
  4. Add a checkbox group with date/time blocks: "Monday 10am–12pm", "Tuesday 2pm–4pm", "Thursday 6pm–8pm", etc. Participants check the windows they can make.
  5. Add an optional checkbox for "I agree to an NDA if required for this research." Make it optional; not all sessions need NDAs.
  6. Export the roster and have your team send session invitations with call links, instructions and any NDA. No automatic scheduling happens.

What goes wrong with this form

Specific to a ux research session signup form, not general advice about forms.

Not specifying device or browser clearly

Do not ask "What do you use?". Ask "Device?" (iPhone, Android, tablet, desktop?) and "Browser?" (Chrome, Safari, Firefox?). Specific questions get specific answers your team can act on.

Offering too many time slots

More than 5–6 slots are overwhelming. Pick the 3–4 time windows you actually have open and list those. If all slots are booked, close the form.

Asking for NDA agreement in the signup form

A checkbox for "I agree to NDA" in the signup is not a legal agreement. Make it optional, mark it as acknowledgement only, and send the real NDA as a document the participant reviews and signs later.

How do you find participants for a UX research session?

Post a form with session type, device, and availability windows. Participants sign up, and your team uses the CSV to pick who to invite based on device and availability. Send session invitations outside the form with call link, instructions, and any NDA.

Can the form automatically assign people to sessions?

No. Your team reads the CSV, picks participants by device and availability, and sends them invitations. Assignment happens outside the form.

What if a participant cannot make any of the listed time windows?

They will not see a matching option and will likely not complete the form. If you want flexibility, add an "Other — I will work around your schedule" checkbox or dropdown option.
