What goes wrong with this form

Making screening too long
Keep screening to 2-3 questions that actually determine eligibility. A form with eight questions gets abandoned. If you have optional screening, make that clear and put it at the end.
Asking open-ended eligibility questions
Use radio buttons or dropdowns for yes/no or categorical questions. Free text produces variations — "employed full-time", "employed FT", "work 40 hours a week" — that you have to manually parse.
Not documenting who was screened vs. enrolled
Export the CSV from the form and keep it with your enrollment records. Your audit trail should show how many people screened and how many were selected — that is part of good research documentation.
