Research form templates

Research forms carry a burden other forms do not: answers have to be comparable across participants, and consent has to be explicit. These templates use fixed scales and required checkboxes where a study needs them, so the export is analysable without recoding.

Whether a form is appropriate for your study depends on your institution, so check with your IRB or ethics office before you recruit. Responses export to CSV, and nothing is shared beyond the destinations you add.

Which research template should you start from?

These differ less in their fields than in what has to be true after someone submits. That is the useful way to choose.

If a researcher needs to document informed consent from study participants

Start from the survey consent form. The form includes a heading and paragraph for study description at the top, and three required checkboxes: understanding the study, age confirmation, and agreement to voluntary participation. The main limit is honesty about what this is — this records consent for your research files, not a substitute for IRB approval where one is legally required, and a typed name is a typed name, not a signature.

If a research study needs to screen potential participants on eligibility criteria

Start from the research participant screening form. Screening questions are collected as radio buttons and dropdowns for yes/no and categorical choices so filtering is clean, availability is a dropdown so you can see who fits your schedule, and participation history is radio. Selection is yours — the form is screening intake only, and your team exports to CSV to decide enrollment.

If a product team is recruiting and selecting usability test participants

Start from the usability testing signup form. Device type is a dropdown so you can filter for the right mix of phones and desktops, experience level is radio buttons so you can balance test participants, and availability is checkboxes so you know immediately who can attend. Scheduling and incentive details stay in your selection email — the form is recruitment intake only.

If a team runs structured research on feature satisfaction and needs to trend which product areas are driving feedback

Start from the product feedback research form. Add a feature dropdown so respondents isolate their feedback to specific areas, a satisfaction slider so you can average scores by feature, and two text areas for what worked well and what needs improvement. The form's main limitation is that it collects research input only — your team still analyzes the responses and decides what to ship.

If you need to recruit software testers who own specific devices and can commit time

Start from the beta tester signup form. Add platform (iOS/Android/Windows/Mac/Web), experience level (first time/occasional/regular), and availability windows (weekly/monthly/flexible) as dropdowns and radios so your team can triage by device and commitment. You can export the roster and invite selected participants, but NDA and access terms are handled by your team outside the form.

If you are shipping a physical product and need to find consumers willing to test it and give feedback

Start from the product testing application form. Collect usage frequency (daily/weekly/monthly/rarely) as a dropdown, current products and opinions as a textarea, and optional checkboxes for written and photo feedback. Your team reviews the CSV to decide who to invite and arrange shipment; the form does not process compensation or selection.

If you have a satisfied customer and want to ask if they will share their story and metrics in a case study

Start from the case study participation form. Include how they use the product as a required textarea, results/metrics as optional, and radios for quotability and interview willingness. The actual case study—interviews, drafts, approvals—happens in conversation with the customer after the form; the form is just permission and initial facts.

If you are running a clinical study and need to pre-screen people for basic interest and availability

Start from the clinical study interest form. Collect condition area and age range via dropdowns and availability windows in a third dropdown so the team knows who to call and when. A text block must state: "This form collects interest only; a study coordinator conducts actual eligibility screening and informed consent."

If you are recruiting volunteers for a university lab study and need to filter by student status and relevant background

Start from the academic study volunteer form. Use a radio for student vs. community status, a textarea for relevant background (languages, prior experience), and a dropdown for time commitment so the team can quickly filter qualified candidates. Actual enrollment and IRB consent are handled by the research team outside the form.

If you want to build a standing research panel so you can repeatedly survey the same audience without re-recruiting

Start from the panel recruitment form. Add demographic dropdowns (age, income, occupation) and a checkbox group for topic interests (technology, health, food, travel, finance, politics) so your team can segment the panel later. The form collects the standing list; your team sends targeted survey invitations filtered by demographics and topics.

If researchers, journalists or students need to request one-on-one interviews with experts or organizations

Start from the research interview request form. Collect purpose and topics as textareas, format preference (phone/video/in-person/email) as a dropdown, and an optional deadline date field. Recipients can accept or decline the request and negotiate scheduling outside the form; the form is the request only.

If you are recruiting applicants to join a paid or unpaid survey panel and need to filter by demographics and interests

Start from the survey panelist application form. Add demographics via dropdowns (age, occupation), topic interests as optional checkboxes, and time commitment as a required dropdown so your team can match panelists to surveys. Applications go to your team for review; acceptance and any incentives are handled outside the form.

If you are running UX research sessions (usability tests, interviews, card sorts) and need to schedule participants by device and availability

Collect research area and how they propose to contribute (funding/data/personnel/access) as textareas and checkboxes, and an optional URL field for prior work so your team can evaluate fit. The form is an inquiry only; no partnership, funding commitment or data-sharing agreement is created by the form.

If a research team needs volunteers to register for studies with self-reported age range and availability

Start from the research participant sign-up form. It collects name, email, age range from a dropdown, availability via checkboxes (weekday morning/afternoon, weekends), and optional study notes. Your team selects participants after review. Age range is self-reported and unverified; consent is collected separately under your team's own process.
