Consultation request form
A consultation request form qualifies which leads are worth a call. This one asks the prospect what they need — as text, not a chat — and when they can talk, so every callback is already scheduled. The form collects the intake; a person still calls back to confirm. No meeting is booked here; the form is the intake step, not the calendar step.
The form beside this text is live. Fill it in and submit it — it validates exactly as it would for a real respondent, and saves nothing.
Preferred contact time is a select field, not a free answer
A dropdown narrows the availability window to times you actually work, so every callback is aimed at a prospect who said they would be available then, instead of guessing when someone picked up.
Area of interest groups requests before you read them
A dropdown of consulting types or service areas means requests for different services sort themselves before your team reads them, instead of one person handling all incoming types.
No card required.

Live previewHow to build this form
5 steps in the editor. Nothing here needs a paid plan.
- Add short text and email fields for the prospect's name and contact details.
- Add a phone field and a select field for preferred contact time (morning, afternoon, specific timezone if relevant).
- Add a required dropdown for the area of interest — consulting type, service focus, or problem category — so requests arrive already grouped.
- Add a long text field for a brief description of the need. Mark it required so every request carries enough detail for a real conversation.
- In Settings, add a webhook or email so each request reaches your team immediately, then review and call back.

What goes wrong with this form
Specific to a consultation request form, not general advice about forms.
Asking for interest as free text
A text field produces "need help", "not sure yet", and vague descriptions from prospects who have not thought it through. A dropdown of service types is what makes the request readable without a follow-up email asking what they actually want.
No preferred contact time field
Without knowing when a prospect can talk, every callback is either a surprise or a voicemail. A select field for morning, afternoon, or timezone-specific times turns every callback into one the prospect said they would be available for.
Making the need field optional
An optional text field leaves some requests with no description of what the prospect actually wants to discuss, which means the callback opens with a question instead of a conversation. A required textarea ensures every request carries enough detail for a real intake call.
Questions about this form
How is a consultation request form different from a contact form?
A contact form is for any message; a consultation request is one type of message with extra screening. This form qualifies by asking what the prospect needs and when they can talk, so your sales or consulting team only calls back leads that meet a minimum bar.
Can I book a meeting directly from the form, or do I still need to call back?
You still call back. The form collects the inquiry and preferred contact time, but a person schedules the actual meeting. No calendar is built in; the intake happens here, confirmation happens on the phone.
What happens if the prospect's preferred time doesn't work for us?
You call back and offer the times you work. The field narrows the initial pool to times your prospect said they could talk, so you are calling them as promised instead of guessing. If the time does not work for you, the conversation adjusts from there.
