Franchise inquiry form
A franchise inquiry qualifies whether a prospect is plausibly investable before you spend time on a pitch. Location, capital and background as structured fields mean the franchisor can compare candidates by the same criteria across every inquiry. This form qualifies interest; it grants no franchise rights. Financial verification, territory approval and terms all happen separately, after someone reviews the intake.
The form beside this text is live. Fill it in and submit it — it validates exactly as it would for a real respondent, and saves nothing.
Capital available is a select range, not a guessed detail
A dropdown of capital ranges lets the franchisor see at a glance whether a prospect meets the minimum investment — which is the barrier for most franchisees — without a read-through of every application.
Target location surfaces market interest by territory
A dropdown of cities or regions lets the franchisor spot geographic clusters of interest and decide where to focus recruitment, or where demand is already met.
No card required.

Live previewHow to build this form
5 steps in the editor. Nothing here needs a paid plan.
- Add short text, email and phone fields for the prospect's name and contact details.
- Add a required select field for target city or region, so the franchisor can see which territories draw interest.
- Add a select field for liquid capital available in ranges (under $50k, $50–100k, $100–250k, over $250k), so the franchisor can compare against minimum investment requirements.
- Add a required long text field for prior business experience, so the franchisor sees whether a candidate has run a business before.
- Add an optional select field for preferred brand or type if the franchisor runs multiple brands, so inquiries come in pre-sorted by which model each prospect wants.

What goes wrong with this form
Specific to a franchise inquiry form, not general advice about forms.
Asking for business experience as optional
A franchisee with no business background is a higher risk and a higher support cost. A required text field asking for prior experience means every applicant describes what they have done, so the franchisor can factor that into their review.
Capital as free text instead of ranges
A prospect typing "$100k" and another typing "around 100 grand" are identical but look different in a list. A select field of capital ranges is what makes every application comparable at a glance.
No location field, or location vague
Without a specific city or region, the franchisor does not know whether a prospect wants a market that is saturated or one that is open. A select field for target territory is what lets the franchisor spot interest clusters and make territory decisions.
Questions about this form
Does completing this form mean I can open a franchise?
No. This form qualifies your interest — location, capital, background — and is the start of the conversation, not an approval. The franchisor will review your application, and if you meet the criteria, they will send materials and discuss territory and terms.
How is capital available different from total net worth?
Capital available is liquid cash you have ready to invest, which is what matters for franchise fees and startup costs. Total net worth includes assets that cannot be easily sold, like a home. The form asks for liquid capital because that is what the franchisor needs to know.
What happens after I submit the inquiry?
Your inquiry is stored and reviewed by the franchisor's business development team. If you meet the initial criteria, they will contact you by email or phone with the next steps, which usually means sending franchise disclosure documents and scheduling a call.
