Site visit request form

A site visit request captures what a contractor needs to know before showing up — address, time, reason and access instructions. A form with all of that makes the visit efficient on both sides. A person still confirms the appointment after receiving the request. The form is the intake; confirmation is a conversation the contractor has with the site contact.

The form beside this text is live. Fill it in and submit it — it validates exactly as it would for a real respondent, and saves nothing.

Time window is a select, not a time-of-day guessing game

A select field for morning, afternoon or a specific hour range eliminates back-and-forth confirming whether 10am is fine for someone who said "afternoon". The contractor and site both see the same window.

Reason for visit tells the contractor what to prepare

A select field for inspection, repair, estimate or installation means the contractor brings the right inspection kit, repair materials or measurement tools, instead of showing up unprepared and having to reschedule.

How to build this form

5 steps in the editor. Nothing here needs a paid plan.

  1. Add a text field for the complete site address including building and unit number, so the contractor arrives at the correct door.
  2. Add a date field and a time select field (morning, afternoon, specific hour if possible) for the preferred appointment window.
  3. Add a required select field for the reason for visit — inspection, repair, estimate, installation — so the contractor can prepare the right tools and forms.
  4. Add short text and phone fields for the site contact person who will be present or available during the visit.
  5. Add an optional long text field for access instructions — gate codes, parking details, whom to ask for — so nobody wastes time looking for the door.

What goes wrong with this form

Specific to a site visit request form, not general advice about forms.

No reason for the visit

Without knowing whether this is an inspection, repair, estimate or installation, the contractor shows up with the wrong expectations and tools, then has to reschedule. A required select field is what makes them ready before they leave the office.

Time window as free text

A prospect typing "morning" and another typing "before 11" and a third writing "ASAP" create three different assumptions about when to show up. A select field narrows it to times the contractor actually works and the site actually means.

No on-site contact person or phone

If the contractor cannot reach anyone when they arrive, they are stuck outside waiting or leave without doing the work. A required name and phone number for who will be present is what keeps a visit from becoming a wasted trip.

Questions about this form

Is the appointment confirmed the moment I submit the request?

No. The form records your request with the preferred time and reason, but the contractor still calls or emails to confirm that the time works for them. The form is the intake; scheduling happens on the confirmation call.

What should I put in the access instructions?

Anything that saves the contractor time looking for the right entrance: gate codes, building parking, instructions like "ask for Sarah at the front desk" or "the side door is closer than the main entrance." If there is nothing special, leave it blank.

Can I request a specific time instead of a morning or afternoon?

Yes. The form offers both options — a time select field and access instructions. If you need a specific hour, put it in the time field or notes; the contractor will see it and confirm whether it works for their schedule.
