Equipment rental request form

Equipment rental starts with one question: what equipment and for how long. This form keeps that the same for every renter — a few fixed fields about what is being rented, when, and how it gets there. Availability, pricing and deposit are what your rental team confirms after the inquiry arrives.

The form beside this text is live. Fill it in and submit it — it validates exactly as it would for a real respondent, and saves nothing.

Rental dates as structured fields, not ranges in text

Start date and end date are separate date fields, so your rental team sees exactly how many days are booked. You can sort by rental length or filter by overlap without parsing "March to June" out of a paragraph.

Delivery versus pickup decided in the form

Radio buttons for delivery or pickup mean the renter has committed to a choice before your team calls. This saves the conversation where the renter says "I thought you delivered" and the team has to explain your terms all over again.

No card required.

How to build this form

5 steps in the editor. Nothing here needs a paid plan.

  1. Add short text and email fields for the renter, plus phone so your team can reach them about delivery or special requirements.
  2. Add a text or dropdown for the equipment needed. If you rent five types of equipment, list them as options; if you rent fifty, let the customer type.
  3. Add date fields for rental start and end dates. A customer typing "next week" costs you a call back; two date fields tells your team exactly when the renter needs it.
  4. Add a radio group for delivery or pickup preference. Most rentals need confirmation of address or location before the rental is confirmed.
  5. Add an optional text field for intended use, which helps your team recommend the right size or model of equipment.

What goes wrong with this form

Specific to a equipment rental request form, not general advice about forms.

Letting the customer type the rental length instead of picking dates

Add separate start and end date fields. "About two weeks, I think" tells you nothing — a renter who picks May 10 to May 25 tells your team exactly how many days to quote and how to schedule pickup or delivery.

Not knowing whether they want delivery or pickup

Use a radio group for delivery or pickup. A customer who says nothing defaults to what they hope, and your team has to call back to confirm logistics. The radio group costs the renter one tap and saves your team time.

Treating the form submission as a confirmed booking

This is a request. Your team still has to confirm availability, agree on pricing, and collect payment. A renter who submits a request for equipment you do not have or dates you cannot cover has not actually rented anything yet — your team needs to follow up with alternatives or a decline.

Questions about this form

What do I ask in an equipment rental form?

Equipment needed, start and end dates as separate date fields so the rental length is clear, delivery or pickup preference, and contact details. The renter decides which, so your team knows whether to plan delivery or confirm a pickup time. Pricing and availability are confirmed by your team on the call.

Can the form check equipment availability?

No. This form logs the request — what equipment the customer wants, when they need it, and how it gets there. Your rental team checks inventory and pricing based on the dates, then follows up with confirmation or alternatives. The form is the intake, not the availability check.

Should I charge a deposit through the form?

No. The form collects the inquiry. A dedicated checkout or a call is where the deposit and payment happen. If the customer never confirms the rental, you did not charge them, and when they do confirm your team collects payment knowing the exact dates and equipment.
