Not giving enough notice
Check the lease for the required notice period — 30 days is typical, but some are 60. Giving less notice than the lease requires does not prevent you from leaving, but the landlord may charge you for the remainder of the lease.

Assuming the form replaces the lease notice requirement
Check the lease for what "proper notice" means. Some leases require certified mail or a notarized letter. This form is a record, not a substitute for following the lease.
