Real estate and property form templates
Property forms mostly move information between tenants, landlords and agents: an application, a maintenance request with the unit and urgency, a viewing request with the times that work. These templates keep those details in their own fields so nothing has to be chased in a reply.
They are intake, not agreements. A form here does not amend a lease, run a background check or satisfy a legal notice requirement in your jurisdiction, and the templates say so where it matters.

Try it liveWhich real estate template should you start from?
These differ less in their fields than in what has to be true after someone submits. That is the useful way to choose.
If you need to screen before you meet anyone
Start from the rental application form. Its consent checkbox is required before submission, and income and occupant counts stay numeric, so applications sort and compare without you reading paragraphs.
If a broken thing needs a place to land
Start from the maintenance request form. Unit number is required, category is a dropdown and urgency is a radio choice, so a report routes to the right trade and the right priority the moment it arrives, with a photo attached if one helps.
If a tenant is staying, not applying
Start from the lease renewal form. A renew/not-renewing radio and a lease-term select capture intent — the FAQ is explicit this is not a binding lease amendment, unlike the rental-application-form for a brand new tenant.
If a past landlord needs to vouch for a tenant
Start from the landlord reference form. Like the reference-check-form for job candidates, it is filled out by a THIRD PARTY, and the FAQ says plainly this is one opinion, not verified fact.
If board members triage complaints by type
Start from the HOA complaint form. Category is a dropdown, the date is a date field with an optional time, and a photo can be attached, so complaints are comparable instead of a stack of emails.
If you want to know who already has an agent
Start from the open house RSVP form. A yes or no radio records whether the visitor is working with an agent, and an optional time slot and guest count help you plan the showing.
If an owner wants a price and you want the seller signal
Start from the home valuation request form. Property type and the reason for asking are dropdowns, so a curious owner reads differently from one about to sell. An agent, not the form, produces the estimate.
If an owner is comparing management companies
Start from the property management inquiry form. Property type is a dropdown, occupancy is a radio and the services wanted are checkboxes, so you see the scope of the job in one row.
If permits differ for residents, visitors and staff
Start from the parking permit application form. Permit type is a dropdown, plate and vehicle are required and proof of residence or employment can be attached. It issues no permit and takes no fee.
If a landlord or matching service needs to screen roommate applicants on move-in date, budget and lifestyle
Start from the roommate application form. Collect applicant name, email, optional phone, required move-in date, budget range dropdown, a required checkbox group for lifestyle preferences (pets, smoking, quiet hours, cleanliness), and optional textarea for personal intro. Limit: this form screens for fit, not credit or background — verification happens separately after serious candidates emerge.
If an HOA architectural committee needs homeowners to submit exterior changes with descriptions and photos
Start from the hoa architectural request form. Collect homeowner name, property address, email, required textarea for the proposed change, optional fields for materials and colors, and optional file upload for plans or renderings. The committee reviews and approves or denies separately outside the form. Limit: photos and specs eliminate guesswork; text description alone is never enough.
If a landlord needs formal written notice from a tenant with move-out date and forwarding address
Start from the move out notice form. Collect tenant name, unit and address, email, current lease end date, required move-out date, optional forwarding address, optional reason dropdown, and required checkbox acknowledging that notice terms comply with the lease. Add timestamps automatically. Limit: this form creates a dated record, not a legal notice — the lease governs what constitutes proper notice in your jurisdiction.
If a landlord or lender needs a prospective guarantor's contact details and consent to be reviewed
Start from the co signer application form. Collect co-signer name, email, required phone, relationship to applicant, employment status dropdown, optional annual income field, and required checkbox for consent to review. Verification of income and credit happens separately. Limit: optional income speeds screening but does not replace verification — a landlord still needs pay stubs and credit reports.
If a real estate agent or landlord needs buyers and renters to request a showing with contact and preferred dates
Start from the property showing request form. Collect buyer name, email, required phone, required property address or listing reference, two optional date fields for preferred times, optional radio for pre-approval status (approved, pre-qualified, not yet), and optional textarea for accessibility or pet needs. The agent confirms by phone or email. Limit: a showing request must include the exact property address or agent cannot route it.
If a tenant needs to formally request early lease termination with a date and reason
Start from the lease termination request form. Collect tenant name, unit and address, email, current lease end date, required requested termination date, and optional textarea for reason. Document the request with timestamps. Limit: this is a negotiation opener, not a guarantee — the landlord can refuse and require payment through lease end, or negotiate a replacement tenant or fee.
If a brokerage needs to qualify leads by capturing intent (buying, selling, or both) and timeline
Start from the real estate agent inquiry form. Collect name, email, required phone, required radio for intent (buying, selling, both), optional dropdown for area of interest, optional dropdown for timeline (next 3 months to exploring), and optional textarea for special needs. This is a lead form; an agent follows up by phone. Limit: no automatic matching or valuation — the agent decides fit and priority.
If a landlord or property manager needs to schedule inspections with type and date preferences
Start from the property inspection request form. Collect requester name, email, optional phone, required property address, required dropdown for inspection type (move-in, move-out, routine, pre-purchase), two optional date fields for the preferred range, and optional textarea for access instructions or special considerations. The inspector produces a separate report. Limit: inspection type frames what the inspector looks for; access must be confirmed or the appointment will not happen.
If a real estate agent needs prospects to request a viewing of a specific property and time
Start from the property viewing request form. It collects name, email, phone, property address or MLS number, preferred viewing date, optional time, and optional questions about the property. You confirm availability in a follow-up call. Does not hold or check calendar slots.
