What goes wrong with this form

Asking open questions you do not need answers to
Do not ask "What device do you use?" as a text field and then wonder why replies say "both" and "whatever is convenient". Use a dropdown for device, a radio group for experience, a dropdown for availability. Closed options are what let you triage the roster.
Not asking about device at all
A beta for iOS needs testers with iPhones. A text field asking "Do you qualify?" is useless; a dropdown asking "Which platform?" is what you need. Self-selection is faster than follow-up questions.
Treating a signup form as an enrollment system
The form collects interest and initial facts — it does not enroll anyone, issue access tokens, send NDAs or create accounts. Your team reviews the roster and handles all of that outside the form. Say so in an FAQ so testers understand the next step.

If you are running a beta, should signups go to a form?
Yes. A form collects structured responses — device, experience level, availability — so your team can triage the beta roster without reading email replies. Each signup still arrives in Slack or a webhook, and the CSV export is the source of truth for who you invited.
Can you prevent someone from signing up twice?
No. The form has no account system, so the same person can submit multiple times. Check for and de-duplicate email addresses in the CSV export before you send invites — that is the stage to catch it.
What happens after the tester signs up?
The form delivers the signup to your team via webhook or Slack, and stores it so you can export the CSV. Everything after that — NDA, access token, test instructions — your team handles outside the form.
