What goes wrong with this form

Asking for device type as free text
Use a dropdown. Free text produces "iPhone", "Apple phone", "iOS device" and "mobile phone" for one category. A dropdown puts everyone in one column.
Not asking about experience level
A new user tests differently than an expert. Ask if they are familiar with your product or category so you can mix experience levels if your test plan calls for it.
Collecting incentive interest on the form, not separately
An optional checkbox for interest is fine, but incentive amounts, payment methods and timelines belong in your selection email or a separate contract, not in the form.
