What goes wrong with this form
Specific to a brand ambassador application form, not general advice about forms.
Letting applicants type their platform name
Use a dropdown for platform choice and a text field for the handle. Free-text platforms produce "Insta", "Instagram", "IG" and "Instagram DM" in the same column, which makes filtering and counting impossible.
Asking for follower count as text instead of a number
A number field validates in the browser and lets you sort by size later. Free text produces "50k", "fifty thousand", "50,000" and "50000", none of which add up cleanly when you export.
Collecting every platform they are on
Ask for their MAIN platform, not every social account they hold. If you want secondary platforms, ask for them in a second field, but the first one should be the platform where they are most active — that is the number you actually need to decide.

What should I ask a brand ambassador applicant?
Name, email, main social platform and handle, follower count, why they want to represent your brand, and optionally a link to their best content. Those six fields tell you platform, audience size and intent. Selection and terms stay yours — the form is intake only.
Can applicants upload their portfolio to the form?
They can supply a URL pointing to their work — a link to their best Instagram post, their YouTube channel, or a website. If you need actual files uploaded, add a file upload field and you can download them from the CSV export or the dashboard.
How do I handle applicants who do not fit our brand?
