Influencer collaboration request form
A brand taking inbound collaboration pitches needs to see the creator, the audience size and the idea all together. This form collects those three things and nothing more — selection and terms are yours to decide separately. The same form handles partnership offers from one-person creators and from small agencies looking to represent multiple accounts.
The form beside this text is live. Fill it in and submit it — it validates exactly as it would for a real respondent, and saves nothing.
Capture audience size as a number, not free text
A number field for followers means you can sort and filter pitches by reach. Free text produces "50k followers", "50000" and "50k" in the same column — structured data lets you work at scale.
Store every pitch in one place for team review
Every collaboration request lands in your dashboard and exports to CSV. Your marketing team can review them together, compare ideas and budgets, and respond to creators externally from your own email.
No card required.

Live previewHow to build this form
5 steps in the editor. Nothing here needs a paid plan.
- Add text and email fields for the creator's name and contact information.
- Add a dropdown for the social platform where they are most visible — Instagram, TikTok, YouTube, etc.
- Add a number field for their approximate audience or follower count, so you can filter by reach.
- Add a long text field asking what kind of collaboration they are proposing — a sponsored post, a long-form partnership, event coverage, etc.
- Add an optional text field for their rate expectations or budget, so you can see if alignment is possible before a conversation.

What goes wrong with this form
Specific to a influencer collaboration request form, not general advice about forms.
Asking for audience size as text
Use a number field. Free text produces "100k", "100000", "100k followers" and "~100k" in the same column, which you have to parse manually. A number field stays clean and sortable.
Collecting every platform they are on
Ask for the platform where they have the biggest audience or the one most relevant to your brand. If you want secondary platforms, ask separately, but leading with "which platform?" keeps the data focused on what matters for the collaboration.
Not asking about expectations early
An optional field for rate or budget expectations saves everyone time. You do not need to commit to a number, but seeing if there is potential alignment before a conversation saves back-and-forth with pitches that cannot work.
Questions about this form
What should an influencer collaboration form ask?
Creator name and email, their main platform and handle, audience size as a number field, what kind of collaboration they are proposing, and optionally their rate or budget expectations. Selection and partnership terms stay yours — the form is intake only.
How do I respond to collaboration pitches?
Every pitch lands in your dashboard and exports to CSV. Review the list with your team, then respond to creators externally from your own email. The form does not send acceptance or rejection letters — you own that conversation.
Can I add more information to the form later?
Yes. Edit the form anytime to add fields like availability, preferred deliverables, or previous brand partnerships. Existing responses keep their data and new responses include the new fields.
