What goes wrong with this form

Asking too many screening questions
Keep screening to demographics and availability that actually determine fit. A form with eight dropdown questions gets abandoned before the submit button — keep it to three or four and add optional open text at the end for detail.
Not asking about availability at signup
Collect time slot availability on the form, not in a follow-up message. If you have preset sessions, ask "which of these dates work for you?" and you immediately know who can actually show up.
Promising participation without selection language
Your form says "sign up" not "apply for a guaranteed spot". Make it clear in the instructions that this is a screening step and not everyone will be selected. Respondents understand that selection will happen, but vague language sets up awkward follow-ups.
