Affiliate application form

An affiliate application vets people who want to join an affiliate program. Website or social media handle, audience size and how they plan to promote tell the company whether this is someone they want representing them. Approval and payout terms are decided by a person reviewing the application, not by automatic rules. This form is the intake; underwriting happens separately.

The form beside this text is live. Fill it in and submit it — it validates exactly as it would for a real respondent, and saves nothing.

Audience size as a select range makes vetting instant

A select field for audience size ranges lets you see immediately which applicants meet your minimum reach threshold, without reading a field claiming "huge list" or "very engaged" that means nothing measurable.

Promotion method checkbox group shows how they promote

Checkboxes for blog, email, social and podcast show the applicant's actual channels, so you can reject someone who plans to promote via a channel that is not your audience or does not fit your brand voice.

No card required.

How to build this form

5 steps in the editor. Nothing here needs a paid plan.

  1. Add short text and email fields for the applicant's name and contact details.
  2. Add a URL field for their website or a text field for their social media handle, so you can actually visit the place they plan to promote from.
  3. Add a select field for audience size in ranges — under 10k followers, 10–100k, 100k–1M, over 1M — so you can compare against minimum audience requirements without reading every field.
  4. Add a checkbox group for promotion methods the applicant plans to use — blog, email list, social, podcast, etc — so you know whether their approach fits your brand.
  5. Add an optional select for payout preference — commission per sale, per lead, monthly retainer — so you know what terms the applicant expects before a discussion.

What goes wrong with this form

Specific to a affiliate application form, not general advice about forms.

No way to see their website or social presence

Without a URL or handle, you cannot tell whether an applicant is actually an influencer, a brand fit or someone exaggerating their reach. A URL field is what lets you verify before approving.

Audience size as free text

An applicant typing "big audience" or "very engaged" or "50k followers" creates three different ideas of scale that are not comparable. A select range is what makes size clear at a glance and filterable.

No promotion method field

Without knowing how an affiliate plans to promote, you might approve someone who wants to use channels that do not reach your audience or do not fit your brand voice. A checkbox field for their actual channels tells you whether it is a fit.

Questions about this form

Does filling out the application mean I'm approved?

No. Your affiliate team reviews the application — website, audience and promotion plan — and decides whether to approve you. If they do, they will send you the affiliate link and payout terms.

What if my audience size is smaller than your minimum?

That is a decision the affiliate team makes after reviewing your application. Even with a smaller audience, engagement and audience match can outweigh size. Apply and let them decide.

How much commission or payout will I get?

Payout terms are set by your affiliate team and sent when you are approved. The application asks what terms you expect, but the team offers what your program actually pays, which may differ.
