Add a URL or file field so customers can provide proof right away. Without it, every request needs a follow-up email asking where they saw the price — half your customers will not respond, and you lose the opportunity to match.

Competitor price as free text

Use a number field. Free text produces "$15", "15.99", "a dollar cheaper" — none comparable. A number field keeps the price sortable and avoids having to parse it.

Treating the form as if it applies the discount

This form records the request. Approving the match and adjusting the price in your system are manual steps. Never promise a customer an automatic discount from this form.
