Cake order form
A cake order form captures what you need to build and bake — the occasion, the size, the flavor, allergies — without taking payment or promising a date. You confirm the order, provide a price, and schedule delivery. The form is where the order starts; the conversation is where it gets locked in.
The form beside this text is live. Fill it in and submit it — it validates exactly as it would for a real respondent, and saves nothing.
Flavors and sizes as dropdowns, not free text
A dropdown for flavor and servings means orders arrive with the same spelling and size every time. Free text produces "vanilla", "vanillin", "plain" for one flavor, which you will normalize when baking.
Dietary and allergy information centralized
Collecting allergies and restrictions on the form, not in a followup call or email, is what keeps them from being missed. The baker sees them attached to the order instead of in a separate thread.
This one uses file uploads, which is a paid feature — you can build and edit it on the free plan, but saving it needs an upgrade.
No card required.

6 steps in the editor.
- Add short text and email fields for the customer, plus a phone field marked optional for delivery questions.
- Add a date field for pickup date and a time field for pickup time.
- Add a select field for occasion (birthday, wedding, etc.) and another for servings or size.
- Add a select field for cake flavor, and a text field for message on cake — keep it a short text field with a character note.
- Add a long text field for allergies and restrictions, and an optional file field for a reference photo.
- Add a radio group for pickup or delivery, then respond to each order to confirm the date, price and any special requests.

What goes wrong with this form
Specific to a cake order form, not general advice about forms.
Leaving flavor as free text
A select field with your actual flavors means orders always spell them the same way and you never bake the wrong thing. Free text produces "choc", "chocolate cake", "dark choc" — three spellings of one flavor that you normalize when boxing.
Not collecting allergies
This is the field that costs lives. Collect it on the form, mark it required or prominently optional, and read it before you bake. A text field saying "none" or left blank is exponentially safer than finding out at pickup that someone has a nut allergy.
No space for special requests or notes
Add a text field for "anything else?". A customer wants color or decoration detail, a specific cake board, a small vegan layer in a mostly traditional cake — the field is where they tell you instead of calling later.
Questions about this form
Can this form collect payment for a cake?
No. This form collects the order — flavor, size, date, allergies — and you respond to confirm it, price it, and collect payment separately. For immediate payment at point of order, a dedicated checkout is the right tool; this is for placing the order itself.
Can I lock in a specific pickup date with this form?
The form collects a requested date. You confirm whether that date is available and send a price. It is your response that locks in the date, not the form — the form is the intake, the confirmation is in your reply.
Should I ask for dietary restrictions on the form or by phone?
On the form, as an optional long-text field. Allergies and restrictions are the one thing you cannot forget, and collecting them while the customer is already filling out the form makes it far more likely they will mention something they would not think to email about separately.
