Gift registry form

A gift registry is a shared list where guests see what is needed and mark what they are bringing — the goal is spreading gifts across a list so nobody brings duplicates. This form collects each guest's claim as a response; you export the CSV and share the updated list, because a guest filling in the form cannot see what others already claimed. The form does not integrate with any store's inventory or payment. It is a coordination tool, not a purchasing system.

The form beside this text is live. Fill it in and submit it — it validates exactly as it would for a real respondent, and saves nothing.

A form that coordinates gifts, not one that orders or pays

This form records which item each guest says they will bring, so you can spot duplicates in the CSV. Buying, paying for and shipping gifts all happen outside the form — the form just organizes who is bringing what.

A checkbox group for items, not a fixed dropdown that might not have what you need

Rather than a dropdown of pre-set gifts, this form lets each response be one item. A guest can suggest anything — wine, decorations, a blender — and the registry grows as people add to it.

No card required.

How to build this form

5 steps in the editor. Nothing here needs a paid plan.

  1. Create one response per gift item: item name, category, and who (if anyone) is bringing it.
  2. Add a select field for category (gifts, wine, decorations, etc.) so the list stays organized.
  3. Add a radio or checkbox field asking if the guest claiming it is actually bringing this item.
  4. Add an optional text field for the guest's name if they want credit for the gift.
  5. Share the form link so guests can see what is needed and mark what they are bringing.

What goes wrong with this form

Specific to a gift registry form, not general advice about forms.

Treating the form as if it buys and ships gifts

This form coordinates what guests are bringing — it does not process payment or arrange delivery. Guests buy and bring gifts themselves; the form only records claims and cannot stop two guests picking the same item.

No way to see what is already claimed

Export the responses to CSV regularly and share the updated list with guests so they can see what is still open before they claim something. Without that visibility, multiple people claim the same gift.

No category field, so the list is just a flat list of items

Add a category field — gifts, wine, decorations, flowers — so the list organizes itself. A flat list of 20 items is harder to navigate than categories with 5 items each.

Questions about this form

How do guests know what to bring on a gift registry?

Export the response list to CSV and share it with guests, or paste the registry items back into a shared document. Guests then open this form to mark what they are bringing. The CSV export shows what is claimed and what is still open.

Can multiple people claim the same gift?

The form lets anyone submit, so yes — multiple people could claim the same gift, which defeats the point of the registry. To prevent that, manage the list manually: export to CSV after each claim, remove claimed items, and share the updated list.

Does this integrate with a store or payment system?

No — it is a coordination form, not a purchasing system. Guests decide what to buy based on the list, handle payment themselves, and submit the form to mark it claimed. The form is the coordination tool; buying and shipping are separate.
